Occasionally, some patients will experience an injury to the mouth during treatment. Dental emergencies while wearing braces or other orthodontic appliances generally fall into two categories.
Braces, wires, and other orthodontic appliances can occasionally loosen, bend, or break during treatment. This may happen due to eating certain foods, accidental impact, or normal wear over time.
If you notice loose bands, loose brackets, broken wires, or poking wires, please call Smile Arc Orthodontics during patient hours to schedule an appointment. For the comfort of all patients, orthodontic emergencies cannot be handled on a walk-in basis.
In the meantime, the following tips may help relieve irritation until we can see you:
Common Appliance Issues and Temporary Solutions
- Irritating tie wire: Cover the area with orthodontic wax or gently tuck the wire under the archwire using a clean pencil eraser or Q-tip.
- Loose band or bracket: In many cases, it can remain in place temporarily. Contact our office when you notice the issue so we can schedule a repair if needed.
- Broken or poking archwire: Try gently bending it inward with a pencil eraser. If necessary, clean nail clippers may be used to trim the wire. If discomfort continues, call our office for guidance.
Direct Injuries to the Mouth and Teeth
Direct injuries, such as those caused by sports, falls, or accidents, should be treated seriously, whether or not you are wearing braces.
If you experience a direct injury:
- Apply ice immediately to reduce swelling.
- Contact your family dentist right away.
Your dentist may need to take X-rays to assess the extent of the injury. If a tooth has been displaced, fractured, or knocked out, your general dentist is usually the best first point of contact, as they have the tools and anesthesia needed for emergency dental care.
Once your dentist has addressed the injury, Smile Arc Orthodontics will adjust or replace any affected orthodontic appliances as soon as possible, based on your comfort level and treatment needs.

Frequently Asked Questions
Orthodontic issues can be stressful, especially when they happen unexpectedly. Below are answers to common questions families in San Diego ask about orthodontic emergencies and how to handle them during treatment at Smile Arc Orthodontics.
What qualifies as an orthodontic emergency?
Most orthodontic emergencies involve discomfort rather than danger. Issues such as poking wires, loose brackets, or irritation are common and usually manageable until you can be seen in our office.
When should I call the orthodontist versus my regular dentist?
If the issue involves braces, wires, or orthodontic appliances, our office is the best place to start. If a tooth is broken, knocked out, or severely injured, it’s usually best to contact your general dentist first.
Can orthodontic emergencies delay my treatment?
Minor issues typically do not affect treatment timelines if addressed promptly. Ignoring a problem, however, can lead to delays, which is why we encourage calling our office when something feels off.
Are orthodontic emergencies common with Invisalign® or clear aligners?
Emergencies are less common with aligners, but cracked aligners or discomfort should still be addressed. If an aligner is damaged or no longer fits properly, contact our office.
Who should I contact for orthodontic emergencies in San Diego?
If you’re unsure what to do, start by calling Smile Arc Orthodontics. Our team will guide you and help determine the next best step.
