What Is MARPE and How Does It Work?

MARPE stands for miniscrew-assisted rapid palatal expander. A small appliance is fixed to the roof of the mouth and anchored with several mini-implants (also called TADs, or temporary anchorage devices). You turn a small screw a little each day, following your orthodontist’s schedule, which gradually separates and widens the mid-palatal suture. Over several weeks, new bone fills in the widened space. Because the force is anchored in bone rather than teeth, expansion is more skeletal and there is less unwanted tooth tipping.

Once the target width is reached, the appliance stays in place for a stabilization period while the new bone matures. Your orthodontist then moves the teeth into their ideal positions as needed.

MARPE vs. SARPE

Both widen the upper jaw, but they are different procedures:

  • MARPE is non-surgical and done in our office, using local numbing for the mini-implants. It is generally lower cost and has an easier recovery.
  • SARPE (surgery-assisted rapid palatal expansion) is performed by an oral surgeon, often under general anesthesia, and is more predictable when the bone is very mature or fully fused.

Dr. Ehsani will use 3D imaging to assess your bone and tell you honestly which approach is more reliable for your case. Older patients or those with very dense bone are sometimes better served by SARPE.

Who Is a Candidate for MARPE?

Most healthy adults and older teens with a narrow upper jaw are candidates. Research shows MARPE can successfully expand the palate in many patients into their mid-twenties and beyond, with fewer dental side effects than tooth-anchored expanders. Can adults really get a palate expander?

Yes. Traditional expanders only work in children whose palatal suture has not fused. MARPE uses bone-anchored mini-implants to achieve true skeletal expansion in adults, and studies show good results well into adulthood.

Most patients feel pressure rather than pain, especially in the first days after placement and during turns. Over-the-counter pain relief is usually enough. The mini-implant placement is done with local numbing.

The active expansion phase usually lasts a few weeks, followed by several months of stabilization while new bone forms. Many cases are part of a broader treatment plan that continues with aligners or braces.

MARPE avoids surgery and general anesthesia, but it can fail to expand if the bone is already too mature. SARPE is highly predictable and is often the better choice for older patients or very dense, fused bone. 3D imaging guides the decision.
